Online registration is now closed. You may register onsite as of Wednesday, February 15th at 5:30pm. 

Registration Fees

The registration fee for PCBIC 2012 includes:

• Wednesday's Evening Reception
• Attendance to all Conference Sessions on Wednesday, Thursday and Friday
• Thursday Evening Reception
• Coffee and Tea Breaks, Thursday's Lunch and Friday's Breakfast

Note: Daily attendance registration fees include the food and beverage functions on that day.

The Early Bird Registration Rate will expire at midnight PST on Monday, January 16, 2012. As of Tuesday, January 17, 2012 Regular Rates will apply.

Registration categories are defined as follows:

• Student is anyone enrolled in a secondary or post-secondary educational institution. Proof of enrollment must be provided at the time of registration. Proof consists of a copy of the student's identification card or a letter from the institution's registrar confirming student status. If proof of student status is not provided the Full Conference registration fee will apply.
• Survivor is anyone who has had a brain injury.

Notes:

• The registration fee is determined by when payment is received, and not by the date of registration.

Payment Details

Cheque
If you are paying by cheque, please include the name of the registrant on the cheque and make the cheque payable to “PCBIC 2012 c/o Sea to Sky Meeting Management Inc.” Please see the payment address at the bottom of this page.  Funds are due in Canadian Dollars and must be drawn from a Canadian Bank. A receipt will be sent to the email address provided in this registration once payment has been received.

Credit Card - Visa and MasterCard
If paying by credit card “Sea to Sky Meeting Management Inc.” will appear as the vendor on your credit card statement.

Conference Registration Cancellations, Refunds and Substitution Policy

Refunds will be available until Monday, January 16, 2012 and will be subject to a cancellation fee of $150 per person for all registration categories, except the Survivor category, where a cancellation fee of $25 will apply. As of Tuesday, January 17, 2012 no refunds will be provided, however substitutions can be made until Sunday, February 12, 2012.  All cancellations or substitutions must be submitted in writing to Sea to Sky Meeting Management at register@brainstreams.ca.

Wheelchair Access and Special Assistance
All Conference venues are wheelchair accessible. If you require special assistance for any reason, please include your requirements during the registration process and confirm any details with the Conference Secretariat.

Dietary Requests
Please identify any dietary requirements during the online registration process so that we can make arrangements in advance for your registered food and beverage events.
