Community Health Services provides a range of health care and support services to eligible clients living in the community. Our in-home and community-based services support you to:

• • • improve your health and quality of life, and
    • remain independent and in your own home for as long as possible.

We can also offer you other options based on your needs, such as residential care and assisted living.

Our services add to, but do not replace, your efforts to care for yourself with help from family, friends and the community.
